A graduate of the University of Western Ontario (2004), Dr. Soordhar has dedicated his career to expanding the boundaries of dentistry by integrating both science and art in patient care.  TMJ Dysfunction as “The Great Imposter”:

  • TMJ disorders present with diverse symptoms that mimic other conditions, including headaches, ear problems, neck and shoulder pain, neurological symptoms, and gait disturbances.
  • Patients often see multiple specialists (neurology, ENT, physiotherapy, chiropractic) before TMJ dysfunction is identified, leading to chronic unresolved pain and disability.
  • A global, interdisciplinary approach is essential for diagnosis and management.

 Patient Population and Presentation:

  • Patients range from children (pre-teens) to adults with symptoms including jaw pain, limited mouth opening, jaw locking, headaches, migraines, ear sensations (ringing, fullness), visual disturbances, neck pain, and neurological complaints such as numbness and gait instability.
  • History taking should include consideration of childhood habits (thumb sucking, tongue thrusting), early bite issues, and family history of headaches or TMJ problems.

 Pathophysiology and Neuroanatomy:

  • TMJ is a unique synovial joint with a disc acting as a cushion between the condyle and the temporal bone.
  • Disc displacement leads to altered biomechanics, nerve compression, and autonomic dysfunction via the trigeminal nerve and associated sympathetic fibers.
  • Arthrokinetic reflexes cause muscle spasms and myofascial trigger points referring pain to head, neck, shoulders, arms, and sometimes hands.
  • The trigeminal nerve occupies a considerable area of the brain’s somatosensory and motor cortex, explaining the wide range of symptoms.

 Clinical Examination and Diagnosis:

  • A “60-second TMJ exam” includes Tonge Blade Test and observation of jaw opening pattern (normal, deviation, deflection), and measurement of mouth opening
  • Palpation for joint sounds and muscle trigger points is critical.
  • Differential diagnosis must exclude serious pathologies such as tumors (e.g., squamous cell carcinoma) and neurological disorders. Appropriate referrals to neurology, ENT, ophthalmology, and imaging (MRI, CT) are essential prior to TMJ treatment.

 Radiologic Findings and Disease Progression:

  • Disc displacement initially causes clicking or popping as the disc slips in and out of place.
  • Progression leads to disc locking and crepitus from bone-on-bone arthritic changes.
  • Early intervention in childhood or adolescence can prevent progression and chronic pain.

 Treatment Modalities:

  • Multidisciplinary approach involving dental appliances (splints and ALF for jaw expansion), neurofunctional acupuncture targeting joint capsule, myofascial trigger points, motor points, cervical muscles (trapezius, SCM), and ligamentous structures for neuromodulation.
  • Acupuncture points were carefully selected to avoid needling the joint capsule directly due to high innervation and hypersensitivity.
  • Orthotic devices are used to stabilize joint position mechanically, reducing nerve and muscle dysfunction.
  • Cranial osteopathy and collaboration with vision specialists (for light sensitivity and balance) are part of holistic care.
  • Emphasis on early treatment of dental development including airway assessment, correction of open bites, tongue and lip ties, and habits impacting jaw growth.

 Outcomes and Case Examples:

  • Several case studies demonstrated significant clinical improvements including resolution of gait and balance disorders, reduction in neck and headaches symptoms, and structural correction of facial and jaw asymmetry.
