Identifying Diagnostic “Waypoints” in the Context of Poly-Pathology

The cognitive processes used by experienced clinicians to arrive at a diagnosis have been well studied and are generally understood as an oscillation between two modes of thinking: pattern recognition through illness scripts, and hypothetico-deductive reasoning.

Hypothetico-deductive reasoning proposes and tests hypothetical diagnoses against known criteria (relevant signs, symptoms, timeline, etc.) and proceeds by eliminating competing diagnoses through a deductive process. Pattern identification is central to Chinese medicine clinical reasoning but has limitations in the context of complex clusters of multiple patterns which have developed over many years.

Overlapping symptoms and mixed excess/deficiency presentations can produce an array of acute conditions occurring above a background of chronic constitutional patterns. Arriving at a clear and decisive differential diagnosis can be elusive. In this complex clinical presentation of Chronic Fatigue Syndrome,

Darren Tellier proposes a new diagnostic heuristic framework that seeks to identify clusters of pathology rather than distinct, differentiated patterns. The framework guides the practitioner to work from diagnostic “way points” that can be readily identified without fully understanding their patterns or complex interrelationships.

Speaker

Darren Tellier is an Associate Professor in the Department of Health and Human Services with more than 20 years of teaching and clinical experience. He is an associate professor teaching a spectrum of lecture, lab, and clinical courses in the Acupuncture Program at MacEwan University in Edmonton, Alberta.

Darren has extensive experience in curriculum development and pedagogical innovation in both classroom and lab environments. He has developed and implemented a wide variety of evidence-based teaching and learning methodologies in the acupuncture program, including formal clinical debriefing for student interns, peer assessment processes, and gamification strategies.

He has also provided leadership in the profession by serving as president of the (former) Acupuncture Society of Alberta and as the founding executive director of the College of Acupuncturists of Alberta. At MacEwan University, Darren provided academic leadership across a broad portfolio of health professions programs while serving as Department Chair for five years.
